May WNYIA share your contact information with a participating fire company? *
Your privacy: Authorized WNYIA level-5 administrators use these answers to coordinate volunteer opportunities and track progress. Sharing follows your choice; this form does not subscribe you to marketing or incident alerts. We review records at least every six months and remove identifying details when recruitment follow-up is no longer needed, normally within 12 months after a case closes. You can request access, corrections, withdrawal, or deletion at admin@wnyincidentalerts.com. Restricted backups may retain deleted records until their normal rotation. Please avoid medical information, birth dates, or other sensitive details in comments.

Important: This is an interest questionnaire, not an application for membership. Your responses are not used to screen or reject applicants. Individual fire companies handle their own application, eligibility, training, and acceptance decisions. WNYIA will not automatically send your answers to any department.
